🆔 What is a UDID Card?
The UDID card is a pan‑India identity document for persons with disability (PwD), combining both a disability certificate and a universal ID—known as the Swavlamban Card. It centralizes PwD data and streamlines access to government benefits and schemes.

Permanent UDID: Issued when a disability is lifelong or unlikely to change; marked with .“P”
Temporary UDID: For disabilities that may change (progress/regress). It's marked “T” and must be renewed after a specified period.
By Severity (Colour-Coded) 🌈

Low-vision – visual acuity between 6/18 and 3/60 or limited visual field
Leprosy-cured persons – post-recovery residual impairments
Hearing Impairment – including deaf (≥ 70 dB loss) and hard of hearing (60–70 dB)
Locomotor Disability – affects movement; physical/musculoskeletal impairments
Dwarfism – short stature below 4'10" (147 cm)
Intellectual Disability – limitations in intellectual functioning and adaptive skills
Mental illness – disorders affecting mood, perception, cognition, etc.
Autism Spectrum Disorder – developmental/neurological communication and social differences
Cerebral Palsy – non-progressive motor disorders from early brain injury
Muscular Dystrophy – genetic, progressive muscle-wasting diseases
Chronic Neurological Conditions – such as multiple sclerosis, Parkinson’s, etc.
Specific Learning Disability – dyslexia, dyscalculia, etc.
Multiple Sclerosis – autoimmune condition affecting central nervous system
Speech & Language Disability – difficulties in speaking or language use
Thalassemia – inherited blood disorder
Haemophilia – blood-clotting inability
Sickle Cell Disease – hereditary blood disorder causing abnormal hemoglobin
Multiple Disabilities – two or more disabilities combined (includes deaf-blindness)
Acid Attack Victims – survivors with impairments due to acid burns
Parkinson’s Disease – progressive neurological movement disorder
Blindness – complete lack of sight or very low visual acuity
🖥️ How to Apply
Step 1: Visit the official site:

Step 3: Submit application:
Choose “Apply for Disability Certificate & UDID Card,” fill in personal/disability details, and upload required documents: photo, ID/address proof (Aadhar, voter ID), signature or thumb impression, and, if available, existing disability certificate .
Step 4: Medical assessment:
If you don’t already have a certificate, you're assigned a district-level hospital/CMO office, where a medical board evaluates your disability and issues a certificate and UDID .
Step 5: Card issuance:
A digital card is issued and sent via speed post. You’ll get an enrollment/application number to track the process.
Step 6: Download e‑card:
Use the portal to download your e‑UDID and e‑disability certificate anytime

Extra services:
Apply for renewal of temporary card, report loss of card, or print a new copy via the portal
